| PLEASE NOTE Thank you for inquiring about our Doberman Pinschers. We are dedicated breeders who participate in
various dog related events. We breed occasionally to produce what we believe to be quality Doberman Pinschers that conform
to the breed standard, and are intelligent, healthy, well-adjusted examples of the breed.
When we have litters,
we make sure our puppies are sold to loving, committed homes. We do our best to screen the owners to ensure a good match between
the puppy and the buyer. Before the puppy leaves our home, it has been well cared for, socialized and loved. The puppy does
not go to its new owner as a trained, well-mannered finished product. It is up to you to complete the job we have started.
You are responsible for the further development of the puppy's personality, through structured discipline, training, guidance,
and love. In an effort to ensure that our puppies are going to receive the love and care they will require and deserve throughout
their lives, we ask all potential buyers to complete this questionnaire. Although some questions may seem invasive, and the
form rather long, it is only the puppy's welfare we are looking out for. If you wish to be put on our waiting list, you must
complete and submit it to us as soon as possible. All of our COMPANION puppies are sold on a Limited Registration with
the AKC - without exception, which means that if the dog is bred, the resulting offspring will not be recognized or register
able with the AKC. COMPANION puppies are required to be spayed/neutered.
Because we have been observing them during
their development, and obviously know them better than potential owners, we make the decisions regarding which puppy goes
to which home based on these observations and the results of the Temperament Test. Therefore, it is unlikely that you would
be able to "pick" your own puppy, unless there is more than one that fits your individual needs. Contrary to popular
belief, there is much more to picking a puppy than choosing the first one that comes to you. Prior to taking the puppy
home, you will be given information on training and care of your puppy, a list of the proper equipment and supplies to obtain
prior to bringing your puppy home, and a sample copy of our contract. On pickup day, you will be given :
a completed
contract (which you must sign), a copy of the pedigree health records a bag of puppy food feeding
instructions baby pictures of the litter at various ages pictures of Mom & Dad information on the number
your puppy has been microchipped with . a blanket and toy that smells familiar, and chewies.
These
items are by no means everything the puppy needs, but we include all of these things to make the transition as smooth as possible
for you AND the puppy. This questionnaire serves the purpose of helping us become better acquainted with the people
inquiring about our puppies. It presents a more complete picture of what you are looking for in a dog and to help us match
you as a potential owner with a dog that will, ultimately, turn out to be what you are looking for. This in no way constitutes
a contract on my part that there will be puppy available for you, nor do your responses serve as the sole determinant of whether
or not a puppy will be offered. Filling out and submitting this questionnaire does not lock you into one of our puppies, either.
Once the puppies are born, and we've decided together that one of our puppies is right for you, a non-refundable deposit of
$200.00 will be required. This means that given any unforeseen circumstances, you will be receiving a puppy from said litter
between the age of 10-12 weeks, depending on the maturity of the individual puppy.

If yes, what are their ages. Does anyone in your household have allergies to animals? If yes, to what and how
severe? Are you interested in a male or female puppy? At what age do you wish to get the puppy? What do
you expect the activity level of this breed to be: very high high moderate below average How many hours a day would
the puppy be kept outside? How would he be confined when he is outside? Do you have a fenced yard or suitable
pen? Have you ever house trained a dog before? How long have you lived at this address? Would you be attending
any training classes with this future puppy? Who is the trainer or organization? If yes to the above, at what age would
you start? Which activities or competitions are you interested in? Have you ever returned a pet to the breeder? If so,
what were the circumstances? Have you ever given a pet away? If so, what were the circumstances? Have you ever
taken a pet to a pound or shelter? If so, what were the circumstances? Are you interested in showing in the conformation
ring? Have you ever shown a dog to it's championship? Are you interested in competition in performance events
such as obedience, rally, agility, or schutzhund? Have you shown a dog to any performance titles? Do you intend
